Hospice palliative care offers people who are terminally ill, and their families or caregivers compassionate care and support that allows them to live fully until the end of their life. The purpose of Hazel Burns Hospice is to help our clients to live in comfort with meaning, hope and dignity.

| Who qualifies for hospice support and how do they get referred? |

Anyone who has been diagnosed with a terminal illness living in the area formerly known as the Old City of York can access our services and support, regardless of age, status or community.
Referrals can be made to Hazel Burns Hospice by anyone, even yourself.

| How does the Hazel Burns Hospice support us? |

Our staff and volunteers work together with your community health care and service providers. We train our volunteers for 30 hours on palliative issues and care. We also do community outreach, to inform and educate the general public on hospice and palliative care.
Our experienced case managers assess and monitor the needs of every client and their families, and work closely with the volunteers.
We receive funding from the Ministry of Health Long-Term Care and private Foundations, as well as holding special fundraising events throughout the year. We are also very grateful for the private donations and bequests that are given to us.
